Position SummaryThe Licensed Practical/Vocational Nurse (LPN/LVN) provides high-quality nursing care to residents under the supervision of a physician and/or registered nurse. Duties are performed in accordance with state scope of practice requirements, facility policies, and professional standards of nursing care.
Administrative Responsibilities- Supervise and support Certified Nursing Assistants (CNAs) as directed.
- Coordinate with the interdisciplinary team (IDT) and healthcare professionals to plan, implement, and evaluate resident care plans.
- Complete assigned sections of the Minimum Data Set (MDS).
- Prepare for physician rounds: organize charts, identify concerns, prepare orders, and document visits.
- Report any suspicion of a crime against a resident according to federal/state regulations.
- Complete documentation including forms, reports, evaluations, charting, and studies.
- Participate in QAPI, infection control, safety, and risk management committees.
- Serve on additional committees as assigned by administration.
- Provide nursing care according to scope of practice and facility standards.
- Monitor and promptly report acute changes in resident condition (e.g., confusion, pain, shortness of breath, fever, mobility changes).
- Obtain and document vital signs as needed.
- Participate in resident admissions and provide orientation for new residents.
- Assist with discharge planning to support goals and safe transitions of care.
- Ensure newly admitted residents have all required physician orders.
- Assist with meals, delivering trays and providing feeding support when needed.
- Contribute to individualized, resident-centered care plans with the IDT.
- Provide compassionate care to residents with cognitive decline, trauma histories, or communication challenges.
- Monitor resident weight, intake, and hydration, report significant changes.
- Document all nursing interventions and services using appropriate records and EMR systems.
- Collaborate with hospice providers to deliver end-of-life care.
- Promote fall prevention practices and resident safety.
- Provide first aid and follow emergency protocols as needed.
- Follow all infection control and safety guidelines.
- Administer medications safely within scope of practice, report adverse reactions and medication errors.
- Transcribe telephone/verbal/telemedicine orders as permitted.
- Perform blood glucose monitoring and specimen collection as ordered.
- Assist with ambulation, ADLs, transfers, and personal care when needed.
- Reposition bed-bound or chair-bound residents per care schedule.
- Assess skin integrity, provide preventive skin care, and perform wound care as ordered.
- Provide trauma-informed care tailored to resident needs and triggers.
- Maintain positive working relationships with coworkers and other departments.
- Assist in training, orientation, and development of new staff as requested.
- Attend required in-service education and continuing education programs to maintain competency and compliance.
- Ensure the environment is free from accident hazards.
- Ensure appropriate resident supervision and use of assistive devices.
- Follow all facility safety, PPE, and infection control protocols.
- Properly use facility-provided PPE including gowns, masks, gloves, and lifting/back support equipment.
- Works throughout the nursing unit (nurses’ stations, medication rooms, resident rooms, etc.).
- Experiences frequent interruptions and may encounter stressful or emotional situations.
- Interacts with residents, families, staff, and regulatory personnel.
- May work beyond scheduled hours, weekends, and holidays as necessary.
- May be subject to emergency call-backs.
