Hope Health Hospice & Palliative Care in Wakefield, RI is seeking a Hospice Registered Nurse (RN) Case Manager for a part-time position of 32 hours per week. The RN will provide clinical oversight for hospice patients, conduct assessments, and collaborate with physicians on care plans.
The ideal candidate will have a commitment to patient-centered care, an RN degree, and preferably a BSN, along with licensed status in Rhode Island and Massachusetts. A variety of benefits, including insurance options and educational assistance, are offered.
