We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Friendly Nursing Home in Woonsocket, Rhode Island, United States. As an Evening Shift LPN, you will play a vital role in providing high-quality care to our residents while working closely with our interdisciplinary team.
About the Role:
Evening Shift LPN: You will work as part of a dynamic team to deliver exceptional care to our residents, with a focus on evening shift hours.
Assessment and Care Planning: Conduct comprehensive assessments, develop and implement individualized care plans, and provide direct patient care to ensure the highest level of care and comfort for our residents.
Medication Management: Administer medications, monitor dosages, and record medication administration to ensure accuracy and compliance with facility policies.
Communication and
Collaboration:
Work effectively with other healthcare professionals, including RNs, CNA's, and other support staff to provide seamless care to our residents.Documentation and Reporting: Maintain accurate and thorough records of patient care, including vital signs, medications, and treatments, and report any changes or concerns to the RN or Medical Director.
Responsibilities:
Provide direct patient care, including vital sign checks, wound care, and personal care assistance
Administer medications, perform treatments, and provide other medical interventions as ordered by the physician
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Collaborate with other healthcare professionals to coordinate care and achieve optimal patient outcomes
Maintain accurate and thorough records of patient care, including documentation and reporting
