Pay: $37.08-$40.17/hour
Sign-on Bonus: $5,000

- 6 p.m.Make a Difference Every Day Seven Hills Rhode Island is seeking a Licensed Practical Nurse (LPN) to provide compassionate, person-centered nursing care to adults with intellectual and developmental disabilities and complex health needs in residential settings.
As an LPN, you’ll help ensure individuals receive safe, high-quality care while supporting their health, independence, and overall well-being. You’ll work closely with residential staff, families, physicians, and interdisciplinary teams to coordinate care and promote positive health outcomes.
What You’ll Do:
Provide nursing oversight and monitor individuals' health status, medications, and treatments.
Support the implementation of individualized health care plans and physician orders.
Train and provide clinical guidance to direct support and residential staff.
Monitor changes in health and communicate concerns to RNs, physicians, and the interdisciplinary team.
Maintain accurate medical documentation, medication records, and nursing notes.
Participate in interdisciplinary meetings, care coordination, and quality improvement activities.
Promote infection prevention, medication safety, and compliance with applicable regulations.
Participate in the Nurse On-Call rotation, including occasional evenings, weekends, and holidays.
Provide clinical support and guidance to residential programs and assist with day program coverage as needed.

High school diploma/GED/HiSET required
Standard driver’s license – class C or class DLicensed Practical Nurse (LPN) with current license in RI1 – 3 years of related experience
