Position OverviewWe are looking for a Trauma‑Focused Community‑Based Services Care Coordinator. The coordinator will provide home and community‑based services under the supervision of a clinician, using evidence‑based, trauma‑informed interventions for youth exposed to trauma, especially those at risk or involved in commercial sexual exploitation of children.
Responsibilities- Deliver trauma‑informed individual, group, and family services to children and youth, aiming to reduce high‑risk behaviors and placement disruption.
- Provide psycho‑education to caregivers about impacts of sexual abuse.
- Address needs of youth impacted by CSEC, including trauma, complex PTSD, attachment, anxiety, depression and other behavioral challenges.
- Collaborate with multidisciplinary teams—clinicians, peer mentors, medical professionals, educators—to provide holistic support.
- Participate in 24/7/365 on‑call rotation for crisis intervention and support.
- Maintain timely and accurate documentation in the TFS Electronic Health Record system, ensuring compliance with organizational and regulatory standards.
- Complete required program assessments (pre and post‑tests).
- Provide ongoing risk assessment, safety planning and stabilization for youth during critical situations.
- Assist families with resource development as needed.
- Support implementation and evaluation of individualized recovery goals.
- Support after‑care and discharge planning, collaborating with youth and all providers to refer to relevant after‑care supports.
- Participate in all required consultation and training to maintain a specialized skill set.
- Meet client engagement productivity expectations set by program or supervisor.
- Bachelor’s degree in Social Work, Psychology, or a related field (required).
- Valid driver’s license and reliable transportation (required).
- Bilingual English/Spanish—preferred.
- Minimum of three years experience providing trauma‑informed care.
- Experience working with at‑risk youth and families preferred.
- Experience with foster care/families formed through adoption.
- Strong understanding of family dynamics, youth development, and evidence‑based therapeutic approaches.
- Ability to work independently and as part of a multidisciplinary team.
- Excellent communication, organizational, and problem‑solving skills.
- Ability to work collaboratively within a multidisciplinary team and with external partners.
- Flexible and adaptable to provide culturally competent and client‑centered care.
$40,000 – $50,000, determined based on experience, education, language capacity, skills, and other relevant qualifications.
