Admissions Director

The Avenue at Wooster is a Post-Hospital Rehabilitation and Skilled Nursing Care Facility that offers a distinctive environment not commonly found in skilled nursing facilities. From our hotel-like environment to our all-private suites, the Avenue is committed to providing an excellent level of physical, medical, emotional, and spiritual support in an environment of comfort and respect. Our facility provides some of the area’s highest level of rehabilitation, nursing and medical support to individuals needing post-hospital care and services.

We are looking for a committed individual with a reputation for being cheerful and friendly as they administer care to our residents.

• High School Graduate with additional training and or experience in long term care or recent college graduate looking to work in the long-term care industry.

• Gather sufficient pre-admission data to determine resident level of care and the ability of the facility to properly care for the resident.
• Interview potential residents and resident's representatives to collect accurate data.
• Work closely with the business office to ensure insurance information is accurate.
• Assign room numbers to residents in consultation with the nursing department and administration.
• Greet and escort residents to their assigned units.
• Follow admission, transfer, and discharge policies and procedures.
• Keep resident's information accurate and confidential.
• Notify appropriate departments of admission.
• Explain facility philosophy, policies, and procedures to potential and new residents and their representatives.
• Complete admission paperwork with resident and/or resident's representatives.
• Work closely with the Director of Business Development and assist with marketing the facility to maintain an adequate census level.
