Job Summary:
Under administrative direction of Value Based Specialist (VBS) in collaboration with Bloomington practices helps patients understand treatment plans and navigate resources.
Duties/Responsibilities:- Help High need patients understand treatment plans and following provider instructions.
- Care coordination with patients to navigate medical care and non-medical need resources for example food, transportation, housing.
- Assists patients with navigation of transition of care from inpatient to ambulatory and/or follow up appointments from emergency department visits.
- Assists VBS in closing care gaps, scheduling and tracking of annual wellness visits
- Assists Practices in chronic care management
- Communication between providers/patients/VBS/ACO team
- Excellent verbal and written communication skills.
- Excellent interpersonal and customer service skills.
- Excellent organizational skills and attention to detail.
- Excellent time management skills with a proven ability to meet deadlines.
- Proficient with Microsoft Office Suite or related software. Especially Excel
Minimum qualifications :
Registered Nurse (ADN)
At least one year related experience required.
