Position: RN - Admissions (Full-Time)

Admission Care Liaison (Registered Nurse) - Full Time

Location:

Newark, Ohio

Schedule:

Full time | 10AM-10PM | Every 3rd weekend rotation and holiday rotation required.

Compassionate Registered Nurse with experience in hospice admissions, patient assessments, and care coordination across hospitals, facilities, and home settings. Skilled in evaluating physical, psychosocial, and spiritual needs, educating patients and families on hospice services, developing individualized care plans, and collaborating with physicians and interdisciplinary teams to ensure seamless transitions and high-quality end-of-life care. Strong communicator committed to patient-centered care, quality improvement, and team collaboration.

Position Overview
  • Conduct comprehensive patient and family assessments, addressing physical, emotional, psychosocial, and spiritual needs in hospitals, long-term care facilities, assisted living communities, and home settings.
  • Educate patients and families about hospice services and eligibility, supporting informed decision-making and smooth admissions processes.
  • Collaborate with interdisciplinary team members to create and initiate individualized plans of care that align with patient goals and needs.
  • Communicate clinical findings and patient status to hospice physicians to support eligibility determination and certification.
  • Provide ongoing assessments, symptom management, and care coordination for patients in contracted facilities while facilitating transitions between care settings as needed.
  • Partner with physicians, hospital personnel, and care teams to ensure continuity of care and timely discharge planning consistent with hospice principles.
  • Support organizational operations and patient care initiatives by assisting across departments when needed.
  • Participate in quality improvement activities to enhance patient outcomes and service excellence.
  • Mentor and orient newly hired staff, fostering teamwork, professional development, and positive working relationships.
Qualifications:
  • Graduate of an accredited nursing school
  • Registered Nurse (RN) license in the state of Ohio without any board actions
  • Two years acute care nursing experience preferred
  • Certification in Hospice and Palliative Medicine (CHPN) preferred and will be made available to Ohio's Hospice employees who wish to obtain certification.
  • CPR Certified
  • Computer skills sufficient to properly document services and care.
  • Valid driver's license and acceptable driving history.
  • Ability to travel as needed in varying weather and road conditions.
  • Current automobile insurance meeting state minimum requirements.
  • Ability to successfully complete a background investigation and 10-panel drug screening.
