Job Description:
We are seeking an Experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our dynamic Hospitalist Team
. In this role, you will be responsible for providing high-quality, patient-centered care to hospitalized patients across various specialties. You will work in a collaborative, fast-paced environment, assisting with the management of acute and chronic medical conditions, ensuring that patients receive the best possible care throughout their hospital stay.
Key Responsibilities:
- Perform comprehensive physical examinations and take medical histories of hospitalized patients.
- Collaborate with attending physicians, specialists, and hospital staff to develop and implement individualized treatment plans.
- Monitor patient progress, adjust treatment plans as necessary, and ensure continuity of care from admission to discharge.
- Order, interpret, and follow up on diagnostic tests and laboratory results.
- Provide ongoing assessment, diagnosis, and management of patients with a wide range of medical conditions, including but not limited to respiratory infections, cardiovascular disease, diabetes, and post-surgical complications.
- Document patient care accurately and thoroughly in electronic health records (EHR) in a timely manner.
- Provide patient and family education on medical conditions, treatments, and care plans.
- Respond to medical emergencies, including performing necessary interventions and managing acute situations.
- Coordinate with multidisciplinary teams to ensure optimal patient care and smooth transitions of care.
- Participate in hospital rounds, case discussions, and care planning meetings.
- Assist in discharge planning and provide patients with appropriate post-discharge care instructions.
Qualifications:
- Certified Nurse Practitioner (NP) or Physician Assistant (PA) with an active, unrestricted license.
- Minimum of 3 years of clinical experience as a hospitalist or in an inpatient care setting.
- Proficiency in performing physical examinations, ordering diagnostic tests, and interpreting medical data.
- Strong understanding of hospital operations, policies, and multidisciplinary team dynamics.
- Ability to work effectively in high-pressure, fast-paced environments.
- Excellent communication skills with patients, families, and healthcare teams.
Ability to provide compassionate, patient-centered care while maintaining a professional demeanor.
