Shifts: Three 12-hour shifts per week | Every-other-weekend rotation

We are seeking a
Licensed Practical Nurse (LPN) – Crisis Care to join our team in providing compassionate, patient-centered care when patients and families need additional support. This is an opportunity to use your nursing skills in a meaningful way while helping patients experience comfort, dignity, and peace at the end of life.
As a Crisis Care LPN, you will provide
one-on-one bedside care for hospice patients who are actively declining and require an increased level of support. Working under the direction and supervision of an RN, you will play an important role in maintaining comfort and supporting patients and their families during a critical time.
- Provide high-quality nursing care to hospice patients in homes and facilities.
- Provide one-on-one bedside care to patients experiencing an active decline.
- Monitor patient condition and promptly communicate changes to the supervising RN and hospice care team.
- Provide comfort-focused care based on the patient's physical, emotional, and end-of-life needs.
- Collaborate with patients, families, caregivers, and the interdisciplinary hospice team.
- Accurately and timely document patient care and services.
- Support patients and families with compassion, dignity, and respect.
- Provide care in accordance with hospice standards, organizational policies, and patient-centered goals.
- Current
Licensed Practical Nurse (LPN) license in the State of Ohio with no active board actions. - One year of nursing experience preferred.
- Current CPR certification.
- Computer skills sufficient to accurately document patient care and services.
- Ability to safely drive during daytime, nighttime, and inclement weather conditions.
- Valid driver's license with a safe driving record.
- Minimum state-required automobile insurance coverage.
- Ability to successfully pass a background check.
- Ability to successfully pass a 10-panel drug screening.
