Senior Pharmacist - Hospital at Home

An exciting newrole has been created for a motivated and patient centered, senior clinical pharmacist to utilise their clinical skills and work with multidisciplinarycolleges across the Integrated Care System (ICS) to provide co‑ordinated care to patients, reduce hospital re‑admissions,support safe discharge, transfer of care, and improve medicines safety and compliance in Worcestershire new Hospital at Home service. A confident communicator with strong organisational skills, who thrives in a MDTenvironment, you will be instrumental in supporting and developing this new service,to ensure a high-quality service for our patients.
This is a full time role.
Main duties of the jobYou will need to be a confident decision maker, with clearleadership skills, excellent pharmaceutical knowledge and technical skills anda sound understanding of pharmacy service and hospital service.
Lead,deliver, develop, and assess the value of clinical pharmacy services to clinical pharmacy support to Hospital at Home service.
Support the development of good practice and safe medicines management and optimisationsystems across Herefordshire and Worcestershire Integrated Care System (ICS).
Work in collaboration with different organisationsacross the ICSto provide highly effective clinical pharmacy services to patients within Hospital at Home service.
Develop and work within appropriate governance structures for the designated clinical service liaising with ICSt widestakeholders as needed.
Provide clinical pharmacy teaching to relevant staff across all grades and disciplines

Person Specification Additional Criteria- I understand that the role involves 1 day in Hereford and 1 day in Redditch
- GPhC registered Pharmacist
- Masters degree in pharmacy or pre 2000 equivalent
- Diploma in Clinical Pharmacy or equivalent demonstrable experience
- Evidence of extensive post registration study/CPD
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
- Significant knowledge and experience of prescribing and medicines management
- Significant knowledge of the national pharmacy agenda
- Management of resources and delivery of projects
