Position Summary: The Community Health Worker/Patient Navigator (CHW) will work to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services of FHCW and make and follow-up on referrals for external services. CHW provides HIV, HCV, STI's (Chlamydia, Gonorrhea, and Syphilis) and TB screening, prevention, counseling & testing, case management services to health center patients and community resident by performing venipuncture and collect urine.

The CHW will provide patient education and advocacy to assist patients with accessing services of FHCW and appropriate, make and follow-up on referrals for extern services. The CHW also provides education on HIV, HCS, STI's and TB to the staff and clients as well as the Worcester Community via outreach to shelters and residential programs. The CHW must be available to do community outreach during off hours and on weekends when needed.

The CHW will support the HIV team with patient follow-up reminders and will address barriers to care. The CHW will document all services as required and will assist in meeting all program requirements.

Educational Requirements:

High school diploma or GED required;
Bachelor’s degree in a related discipline preferred.

Experience Required:

Minimum of two years relevant work or college-level internship experience required. Data entry skills and proficiency with MS Office including Excel and Outlook. Proven ability required to perform patient interviews, coordinate and collaborate with internal services and external community partners. Must have demonstrated organizational skills and the ability to work independently.

  • Must have a certification in phlebotomy or successful completion of training and certification within six months of placement in position.
  • Bilingual in English and another language appropriate to FHCW patient population required.
  • Community Health Worker Core Training required within first year of employment.
  • Flexible evening work, weekend hours required to meet FHCW community or patient needs.

Monday-Friday; 8:30am
