What You'll Do
- Screen, evaluate, and develop tailored care plans for patients with physical functioning disorders or wound care needs
- Implement therapeutic interventions such as manual techniques, ambulation, exercises, modalities, and use of assistive devices
- Document patient progress, adjust treatments, and evaluate outcomes to optimize recovery
- Communicate effectively with patients, families, caregivers, and interdisciplinary team members
- Assist in discharge planning and provide comprehensive documentation following regulatory standards
- Supervise and support PT assistants, rehab aides, and students when necessary
- Participate in care conferences, meetings, and required training sessions
What You Need
- Valid Massachusetts Physical Therapist license
- 1+ year of relevant clinical experience in LTC, rehab, or outpatient settings
- Strong knowledge of clinical practices, documentation standards, and patient-centered care
- Excellent communication and teamwork skills
