Community Health Network (CHN) has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer a stable, supportive, and growing organization that focuses on your needs and interests.
We are looking for an experienced Home Care Physical Therapist for coverage in the Worcester, Uxbridge, Bellingham MA areas.
- Monday-Fri, 40 hours/week
- No weekends or holidays required
- Highly competitive pay rate plus IRS mileage reimbursement
- Contract
Requirements: 1 year of recent Home Health Physical Therapy experience doing OASIS visits with a Medicare certified agency.
