JOB DESCRIPTION
Job Title:Case Manager
Program:Crozier House
Location:Worcester, Massachusetts
Reports To:Clinical Supervisor
Status:Non-exempt
Primary Responsibility
:
Provides counseling, both individually and in group settings, while being responsible for the overall development, implementation, and evaluation of treatment planning and educational programs for all residents within the Crozier House Residential Recovery Home Program.
Essential Duties:
- Responsible for the daily clinical activities of the Crozier House Program, and the monitoring of the daily living schedules and well-being of the residents.
- Develop, implement, and evaluate treatment plan goals and educational programs for residents using assessment tools and the SNAP Treatment Planning model.
- Advocate for the resident’s rights, benefits, and needs.
- Provide a follow-up for the residents upon discharge from Crozier House.
- Provide information and referral services to residents regarding services available to them in the community (e.g., mental health counseling, medical services, and career services).
- Attend and participate in meetings, seminars, and local conferences for educational and training purposes (keep current with latest developments in substance use treatment).
- Complete required documents, forms, and reports in a professional, accurate, and timely manner.
- Attend required Electronic Medical Record (EMR) orientations, meetings and trainings to become proficient in its use.
- Keep informed and follow all Crozier House policies and procedures, as well as enforce resident clinical policies and procedures.
- Participate in regular staff meetings, supervision meetings, clinical, and treatment team meetings.
- Conduct weekly progress meetings with residents assigned to individual caseload.
- Collaborate closely with Recovery Specialist (RS) staff by adhering to protocols, communicating policies, and acting on the feedback they provide.
- Take the lead, whenever possible, in the event of an emergency, crisis, or medical situations.
Non-essential Duties:
- Admit new residents to the program of Crozier House according to program intake and process.
- Keep clerical and housekeeping supplies up to date.
- Interpret the programs of Crozier House to the community either by phone or in person.
Knowledge/
Experience:
- Minimum of 3 - 5 years of experience in dealing with substance use disorder.
- Minimum of 3 - 5 years of experience in dealing with co-occurring disorders as noted in ASAM 3.1 level of care.
- Experience in the use of 12-core functions of the Substance Abuse Counselor.
Skills/Abilities:
- Ability to read, write and speak English fluently and articulately. Second language a plus (especially Spanish). Able to prepare comprehensive, legible reports.
- Competency with Microsoft Office (Word, Excel, Teams) as well as an understanding of virtual meeting platforms.
- Understanding of the needs of individuals with substance use disorder or a dual diagnosis in crisis, ability to relate in a compassionate, caring, and professional manner.
- Ability to maintain personal boundaries, respect the boundaries of others, and set limits where appropriate.
- Ability to respect the client’s right to privacy and confidentiality, communicate information to colleagues only on a "need to know" basis.
- Ability to use supervision to determine when information needs to be shared with other staff.
- Ability to be flexible, sensitive, and willing and able to support the needs of the residents, colleagues, supervisors, and administrators.
- Ability to cope with difficult decisions, and to ask for support and help when determined to be needed.
- High level of comfort working with diverse people from different cultural backgrounds, ideologies, identities, preferences, socioeconomic status, faiths, life experiences and other traits.
Licenses/Certifications:
- Valid and current driver’s license, registration, and proof of insurance.
- Bachelor’s Degree from an accredited college/university in a related field.
- As a substitute for a Bachelor’s, must be a Certified Alcohol/Drug Counselor (CADC) or LADC certification is strongly suggested at the time of hire, or must be in the process of attaining CADC within the first year of hire, or…
