- Screen, evaluate, and treat patients with communication, cognitive, or swallowing disorders in a long-term care setting
- Develop and implement individualized care plans based on standardized assessments and evidence-based practices
- Monitor patient progress, modify treatment as needed, and document results accurately
- Provide consultation and counseling to patients, families, and caregivers on speech and swallowing concerns
- Collaborate with the healthcare team to ensure comprehensive patient care
- Document all clinical activities in the electronic health record, following all regulatory guidelines
- Participate in meetings such as patient care conferences and team reviews
- Support and supervise CFY or student speech-language pathologists as needed
- Adhere to ASHA and State practice standards, ethics, and conduct throughout your practice
- Valid MA State Speech-Language Pathologist license
- Certification from ASHA or equivalent
- Minimum of 1 year of experience in a healthcare or long-term care setting preferred
- Strong communication, assessment, and treatment planning skills
- Ability to work effectively within a multidisciplinary team
