The Coordinator, Ambulatory Care (CAC) position assists the patients in navigating the mental health care and social systems, working to remove barriers to patients receiving needed supports and services. The CAC is responsible for updating care plans, monitoring the status of recommended mental health services, facilitating referrals and appointments and making connections for patients to community agencies and support services. The CAC helps the providers, nurses and other members of the psychiatry team to know what is occurring on a timely basis with the patient and their status, with a particular focus on patients with multiple social and mental health needs.
I.Major Responsibilities:
- Functions as an integrated member of the psychiatry consultation team to facilitate coordination and follow-up of recommended mental health and social services.
- Advocates for patients and families as they negotiate the mental health care system so that effective care, treatment and planning may occur.
- Coordinates, facilitates and monitors referrals to service providers, including behavioral health, health education, social support and community agencies.
- Extracts information from the medical record in order to facilitate appropriate referrals and other interventions.
- Documents interventions and other information in the patient’s care plan and medical record to ensure that all members of the care team understand the patient’s medical and social status.
- Connects patients and their families to resources to overcome barriers and social problems which may impact their ability to engage in and improve their health.
- Follows up with patients to ensure that services are arranged and delivered; responds to questions from patients about available resources in the psychiatry practice or in the community.
- Works closely with clinical care managers, nursing staff and others to engage the patient in their own health care.
- Meets regularly with the psychiatry consult and outpatient psychiatry leadership teams to review cases, identify next steps, set priorities and plan feedback. Identifies and recommends solutions regarding care processes.
- Assists with the development and updating of community resource lists and patient education materials as required.
- Establishes and maintains effective working relationships with community resources and agencies through telephone, electronic and/or direct contact.
