What You Will Do
  • Provide direct residential support that promotes each individual's safety, growth, independence, decision-making, dignity, and quality of life.
  • Assist and monitor activities of daily living, personal care, household routines, and other individualized supports while encouraging individuals to build skills and confidence.
  • Monitor overall health and well-being and assist with medication treatment, administration, and related documentation in accordance with Medication Administration Program requirements and individual needs.
  • Provide crisis intervention and emergency support as needed, communicate concerns promptly, and follow established safety and response procedures.
  • Implement strategies identified in Individual Service Plans and Person-Specific Treatment Plans and provide ongoing information, reviews, and documentation related to goals and progress.
  • Help individuals strengthen social and natural-support networks, maintain family and guardian relationships, and participate in religious, recreational, and community groups and activities of their choosing.
  • Plan and support meaningful community activities and provide safe, timely transportation for appointments, shopping, errands, recreation, and other individual needs.
  • Prepare nutritious meals and snacks and complete routine cleaning, household maintenance, and safety tasks with or without the participation of individuals served.
  • Complete shift summaries, progress notes, incident reports, medication-occurrence reports, health records, and other program-specific documentation accurately and on time.
  • Recognize and report health, safety, medication, environmental, and maintenance concerns and complete assigned shift responsibilities, including required status calls during awake overnight shifts when scheduled.
  • Participate in meetings, in-service education, and required training; maintain all mandatory certifications and collaborate effectively with individuals, families, guardians, coworkers, and the ACCS team.
Who You Are
  • High school diploma or GED required.
  • At least six months of experience in human services preferred; BHN provides role‑specific training.
  • Valid driver's license and a reliable vehicle available during all working hours required.
