About The Program
The Children’s Community Support Collaborative (CCSC) provides comprehensive mental health services tailored to the unique needs of youth and their families. Our services, which take place at home, in the community and at school, focus on the family as a whole to help youth who have experienced trauma or mental health challenges remain safe within their homes and communities.
Our team prioritizes relationships and gives a voice to youth and caregivers. Each family is served by a core team, including a Master’s-level clinician and Community Outreach Worker, which provides intensive in-home family treatment, comprehensive care coordination, mentorships and connections to informal and formal community-based resources.
For youth who cannot live safely at home, services include therapeutic group care and planned respite care. If such services are needed, our team works closely with families to support youth in returning home as quickly as possible.
How You'll Be Making a Difference
- Administers assessments and applies evaluation techniques to determine functional status and identify therapy needs
- Analyzes evaluation results and designs interventions to meet the needs of client, incorporating factors of child’s developmental and medical history, social environment, and educational placement
- Develops individual treatment plans, adapting and adjusting goals and objectives appropriately
- Provides individual, consult, and group treatment on site at the Therapeutic Group Care setting as well as direct and consult services to youth and families receiving services within their home settings
- Demonstrates effective oral and written communication skills necessary for multidisciplinary team meetings and family meetings as indicated
- Participates as needed in weekly treatment team reviews and quarterly Family Team Meeting sand other meetings, as requested by the youth and/or other members of the treatment team.
- Collaborates with parents, guardians, collateral agencies, community providers, and schools to coordinate client’s treatment needs
- Provides clinical supervision to Occupational Therapy students in internship placements within The Home as requested
- Uses time effectively, maintains treatment notes and completes all documentation in a timely manner
- Alters schedule as needed to meet the clinical needs of the program
- Functions independently within the scope of these duties consulting with the supervisor for administrative issues/departmental operations and case reviews
- Serves on agency–wide and/or program committees as appropriate
- Provides in-service training regarding strategies to facilitate carry over of skills learned in occupational therapy
