Community Health Program Inc seeks a Behavioral Health Intake Coordinator to serve as the first point of contact for patients seeking mental health services. In this role, you will complete phone and in-person screenings, gather histories, assess urgency, and coordinate timely referrals to CHP behavioral health providers. You will verify insurance, schedule appointments, and document accurately in the EHR while collaborating closely with primary care and behavioral health teams.

Responsibilities
  • Serve as first point of contact for patients seeking behavioral health services via phone and in person
  • Conduct structured intake interviews, screenings, and basic risk assessments to determine urgency and appropriate level of care
  • Coordinate referrals and schedule appointments with behavioral health and primary care providers
  • Verify insurance coverage, collect required demographic and clinical information, and obtain necessary consents
  • Document all encounters, screenings, and referrals accurately and promptly in the EHR
  • Collaborate with integrated care teams to support warm handoffs and continuity of care
  • Provide patients and families with information about available behavioral health and support services
  • Prioritize and escal high-risk or crisis situations following established protocols
  • Maintain strict confidentiality and compliance with HIPAA and organizational policies
  • Contribute to a respectful, inclusive, patient-centered environment aligned with CHP's mission and values.

Qualifications
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) preferred
  • Certified Medical Assistant (CMA) preferred
  • Certification in mental health first aid or crisis intervention a plus
  • Familiarity with HIPAA and behavioral health documentation standards
