Schedule for this role is 8A-830p or 730a-8p (3 shifts per week; 12 hour shifts) with every other weekend rotation.

The Case Manager is responsible for delivering comprehensive patient care, including individual, group, and family counseling. Key duties include conducting initial and ongoing clinical assessments, demonstrating expertise in various therapeutic interventions, and utilizing evidence-based practices. The Case Manager will actively participate in the Treatment team, coordinate services for patients as needed, and serve as an advocate by communicating with the Utilization Review (UR) department regarding all aspects of patient care.

This role is integral to American Addiction Centers' commitment to providing the most comprehensive care possible to address issues that may hinder a patient's recovery.

• Provide substance abuse counseling services in accordance with the state laws, licensing boards, and professional ethical guidelines.
• Maintain a full patient caseload.
• Provide intervention, assessments, treatment planning, and individual/group counseling.
• Initiate and provide family counseling services.
• Provide intervention, assessments, and group counseling.
• Facilitate therapeutic groups as assigned.
• Provide clinical services and assist when needed with patients as assigned by the clinical director.
• Participate in daily treatment team meetings and program staff meetings.
• Ensure that all documentation meets contract, licensure, accreditation and standards for services in a timely manner following the AAC’s documentation policy guidelines.
• Communicate efficiently with other departments to ensure the best care for patients.
• Provide clinical services to the outpatient and inpatient adult population.
• Understand and abide by State Department standards, facility accreditation, AAC’s policy and procedures.
• Identify, evaluate, and address situations which may be potentially harmful to patients and report any patient or safety concerns to the Clinical Director and/or Facility Executive Director.
• Adhere to company code of ethics and maintain professional boundaries with patients at all times.
