What You Will Do:
- Provide intensive, community-based clinical services within the Program of Assertive Community Treatment (PACT) model for individuals with complex behavioral health needs, including those involved with the forensic system.
- Complete intake evaluations, clinical formulations, recommendations, risk and crisis evaluations, and individualized safety plans using sound clinical judgment.
- Provide individual, family, couples, and group treatment grounded in appropriate clinical theories, principles, and recovery-oriented practices.
- Serve as primary clinician and case manager for an assigned caseload, completing assessments, treatment plans, reviews, financial-management plans, and strategies that support measurable progress.
- Coordinate care with family members, residential providers, hospitals, community partners, and other members of each individual's support network as clinically appropriate.
- Provide frequent outreach in homes, hospitals, residences, and community settings while encouraging independence, informed decision-making, dignity, and community integration.
- Assist with and monitor activities of daily living, connect individuals with community resources, and support access to medical, behavioral health, social, and recovery services.
- Support medication-related services within role expectations and complete health and medication documentation in accordance with Medication Administration Program (MAP) regulations and agency procedures.
- Complete progress notes by the end of each shift and maintain timely treatment, incident, medication-occurrence, shift-report, and program documentation.
- Maintain direct-service and outcome expectations, participate in shift communication, supervision, meetings, training, and continuing education, and follow PACT policies and procedures.
- Provide safe transportation as needed and maintain flexibility to work assigned weekends, second shifts, and holidays based on program needs.
