• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide therapeutic interventions to improve strength, mobility, balance, coordination, and functional independence.
• Help patients safely achieve their mobility and functional goals in their home environment.
• Educate patients and caregivers on exercises, mobility strategies, fall prevention, and appropriate use of equipment.
• Collaborate with patients, families, physicians, and interdisciplinary team members to coordinate care and achieve optimal outcomes.
• Monitor patient progress and adjust treatment plans as needed.
• Complete timely and accurate clinical documentation in accordance with company and regulatory requirements.
• Maintain current clinical knowledge through continuing education and professional development.

• Current Massachusetts Physical Therapist license.
• Home health or outpatient experience preferred.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and effectively manage a patient caseload.
• Strong organizational and time-management skills.
• Valid driver's license and reliable transportation.
