In this role, you'll provide individualized, patient-centered care in the comfort of patients' homes—helping them regain independence, improve daily functioning, and achieve meaningful goals.

• Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans.
• Provide therapeutic interventions to improve motor skills, cognitive functioning, mobility, and activities of daily living.
• Help patients maximize independence and safely remain in their homes.
• Educate patients and caregivers on adaptive equipment, techniques, and strategies to support daily living.
• Collaborate with patients, families, physicians, and interdisciplinary team members to coordinate care and achieve optimal outcomes.
• Document patient progress accurately and maintain compliant clinical records.
• Maintain current clinical knowledge through continuing education and professional development.

• Current Massachusetts Occupational Therapist license.
• Home health or outpatient experience preferred.
• Strong clinical, communication, and interpersonal skills.
• Ability to work independently and effectively manage a patient caseload.
• Strong organizational and time-management skills.
• Valid driver's license and reliable transportation.
