Salary Range: $50.00 To $150.00 Hourly
Job Summary:
The Registered Nurse (RN) for the Group Adult Foster Care (GAFC) and Adult Foster Care (AFC) programs provides clinical oversight and ensures that all members receive safe, high-quality care in community-based settings. The RN collaborates with case managers, caregivers, and healthcare providers to assess member needs, develop care plans, and monitor ongoing health status while ensuring compliance with all applicable state regulations.
Key Responsibilities:
- Conduct initial and ongoing clinical assessments of GAFC/AFC members in accordance with Massachusetts program regulations
- Develop, implement, and update individualized care plans, including clinical needs and Activities of Daily Living (ADLs)
- Complete required in-home or site-based nursing visits based on program requirements and member level of care
- Provide clinical supervision and guidance to case managers and direct care staff
- Train caregivers and staff on medical tasks, infection control, medication management, and safety procedures
- Monitor member health status and identify changes requiring intervention, referral, or escalation
- Coordinate care with physicians, specialists, therapists, and other healthcare providers
- Ensure compliance with program documentation standards (e.g., nursing assessments, progress notes, care plans)
- Participate in interdisciplinary team meetings and case reviews
- Attend and actively participate in bi-monthly team meetings for care coordination, case discussion, and program updates
- Report suspected abuse, neglect, or exploitation in accordance with guidelines from the Massachusetts Executive Office of Elder Affairs and the Disabled Persons Protection Commission
- Respond to urgent clinical concerns and provide guidance in crisis situations
Qualifications:
- Active Registered Nurse (RN) license in the Commonwealth of Massachusetts (required)
- Graduate of an accredited nursing program (ASN or BSN required; BSN preferred)
- Minimum of 1–2 years of nursing experience; home care, geriatrics, or community health preferred
- Experience with GAFC/AFC or community-based care models strongly preferred
- Valid Massachusetts driver’s license and reliable transportation

Certifications:
- Current RN license in Massachusetts (required)
