• Licenses: RN‑MA
• Certifications:
  
  ACLS; BLS; PALS; CERT‑TNCC
• Include past 7 years of work experience (if less than 7 years of nursing experience, non‑clinical work history must be provided to fulfill 7‑year WH requirement)
• 2 recent references, minimum 1 Supervisor or Above
• Reference must be from same skillset of the order being submitted to
• RN license copy on file
• Education must include city and state
• Can pass fit test: Y/N answered
• Flu vax: on file or willing to obtain (declinations not accepted)
• TNCC

• Charge experience preferred
