Position: Breast Center Nurse Navigator, 40 Hour Days

I. Major Responsibilities:

Facilitates the entry of patients into the diagnostic and/or treatment system and coordinates their care, including family involvement, and follows patients throughout the continuum of health care services across multiple departments.

Assists patients in understanding their diagnosis, treatment options, recommended follow‑up and available resources.

Utilizes the nursing process to assess the patients’ physical and psychological status comprehensively.

Evaluates patient progress along the continuum and is responsible for professional and personal modifications in collaboration with the healthcare team as appropriate, including tracking post‑care patients to ensure post‑treatment standards of care are met and ensuring that patients receive clinical satisfaction surveys.

Coordinates participation in multidisciplinary planning conferences and collaborates in the development of patient summaries for use by the care team.

Education:

Required:

Bachelor’s degree in Nursing.
Required:

Current Massachusetts Registered Nurse licensure and registration.

Preferred:
Master’s Degree in Nursing.

Experience/

Skills:

Required:

Minimum of 5 years nursing experience.
Required:

Excellent teaching, consultation, and customer service skills.
Required:

Excellent critical thinking skills.
Required:

Demonstrated leadership experience.
Required:

Excellent verbal and written communication skills.
Required:

Computer literate with knowledge of Microsoft Office products.

Preferred:
Skilled in the utilization of EMR and other software.
Preferred:
Experience with project management.

Unless certification, licensure or registration is required, an equivalent combination of education and experience that provides proficiency in the responsibilities listed may be substituted.

III. Physical Demands and

Environmental Conditions:

Work is considered sedentary. The position requires indoor work in a patient care and office environment.

Breast Nurse Navigators require accreditation through NAPBC.

Preferred:
Oncology Certification preferred unless a Breast Nurse Navigator.

Experience/

Skills:

Required:

Minimum of 5 years oncology experience in a specific disease site (breast, lung, gastrointestinal, etc.) and post‑surgical nursing.
