LPN: $85-98k per year + $6,000 Travel Stipend
RN: $110k+ per year + $6,000 Travel Stipend

Experience:
You bring a wealth of Nursing experience, preferably in the geriatric, long term care, and Home Healthcare setting. Your understanding of the functions of a Nursing Supervision, and the Admissions processes will make you a valuable asset to our team.

Whether it’s speaking with families, collaborating with team members, or liaising with external partners, your exceptional communication skills and guidance foster strong relationships and build trust.
Must Haves:
- Currently licensed LPN or Registered Nurse in good standing, in the State in which services are provided.
- Graduate of an accredited school of nursing.
- At least two-years of experience in an acute care setting, one of which is in a home healthcare setting is preferred.
- Current CPR certification and must maintain certification.
- Possess effective communication skills and be able to follow instructions to complete work as scheduled.
- Active and current driver's license and have a dependable insured automobile. Must be comfortable with significant daily driving to and from client residences.
- Perform essential activities including assessment, planning, implementation, coordination, monitoring, evaluation, outcomes, and general interventions across case management practice and processes, and make appropriate referrals to other disciplines.
- Perform initial evaluation and assessment of client needs and contact physician for initial/continuing orders.
- Establish the Care Plan with team, coordinate, implement appropriate alternatives, and determine expected client outcomes and goals.
- When assigned, supervise ancillary personnel.
- Participate in team conferences, as requested, and attend staff in‑services programs.
- Provide Nurse Delegation to appropriate staff and supervise same to assure compliance with the delegation and safety of the clients.
- Thoroughly explain the Company’s mission and value to the client and client’s family members, doctors, providers, etc.
- Explain role and services of the Company, describe client’s rights, and provide contact information for complaints.
