Licensed Practical Nurse (LPN) Responsibilities

The Licensed Practical Nurse (LPN) is responsible for, but not limited to:

  • Observe and document the physical, emotional, and psychosocial needs of patients and their families.
  • Collaborate with the Hospice RN regarding patient status, symptom management, and changes in condition.
  • Provide skilled nursing care as directed by the attending physician and supervised by the Hospice RN.
  • Monitor patient responses to treatment and document all care provided.
  • Maintain continuity of care through effective communication with patients, families, physicians, and the interdisciplinary team.
  • Participate in Interdisciplinary Team (IDT) meetings and other required clinical meetings.
  • Maintain adequate home care supplies to support quality patient care.
  • Assist with collecting clinical documentation and required paperwork.
  • Support clinical billing activities as needed.
  • Assist with the orientation and training of new Hospice Aides when assigned.

Education
  • Associate Degree in Nursing or a related field preferred.
Qualifications
  • Current, unrestricted LPN license in the state of practice.
  • Minimum of one (1) year of clinical nursing experience.
  • Hospice, Home Health, or Long-Term Care experience preferred.
  • Strong verbal and written communication skills.
  • Ability to work effectively as part of an interdisciplinary team.
