Brockton Home Health AFC is seeking a registered Nurse (RN)/Case Manager to manage the Adult Family/Foster program. The role covers supervision of Home Visits RN’s, completing MDS-HC and clinical assessments, and developing AFC/GAFC plans of care.
You will train and supervise AFC/GAFC caregivers, monitor member health, and coordinate with physicians. Massachusetts RN license and 5+ years in home health preferred.
