Autumn Hospice is seeking an experienced Licensed Practical Nurse (LPN) to provide skilled nursing care in a hospice setting. You will collaborate with the Hospice RN, monitor patient conditions, and document care while supporting the interdisciplinary team to deliver compassionate end-of-life care.
The role requires an active LPN license in the state of practice and at least one year of clinical nursing experience, with Hospice/Home Health experience preferred.
