Case Manager I - Children's Congurate Care
Pay: $24.00/hour

In this role, you’ll be part of a multidisciplinary team working to improve the mental health and overall well‑being of children and adolescents. You’ll help coordinate services, support treatment goals, and play a key role in ensuring successful transitions back into the community.

- Provide direct case management services to youth in residential care.
- Support implementation of individualized service and treatment plans
. - Connect youth and families to community resources and natural supports
. - Coordinate care with internal teams and external providers (e.g., medical providers, schools, and state agencies).
- Assist in planning for safe and stable discharge from residential care.
- Coordinate and follow up on aftercare services to support continued success.
- Serve as a liaison between families, treatment teams, and community partners
.
- Actively participate in multidisciplinary team meetings and case conferences
. - Communicate updates and concerns in a timely and professional manner
. - Work in the residential milieu as needed to support program operations and youth care
.
- Respond to crisis situations and coordinate appropriate interventions.
- Monitor and address safety risks within the program environment.
- Maintain training in CPI/de‑escalation, First Aid/CPR, and medication protocols
.
- Maintain accurate and timely documentation in Electronic Health Records and required systems
. - Ensure compliance with HIPAA, agency policies, and best practices
. - Track services, progress, and outcomes to support quality care.
- Strong interest in working with youth in behavioral health or residential settings
- Ability to coordinate services and manage multiple priorities
- Strong communication, organization, and problem‑solving skills
- Commitment to trauma‑informed, strengths‑based, and client‑centered care
- Ability to work both independently and as part of a team
- High School Diploma or equivalency certificate.
- Valid driver’s license (Class C or
D)
