Pay: $24.00/hour

In this role, you’ll be part of a multidisciplinary team working to improve the mental health and overall well‑being of children and adolescents. You’ll help coordinate services, support treatment goals, and play a key role in ensuring successful transitions back into the community.

Responsibilities Case Management & Care Coordination
  • Provide direct case management services to youth in residential care.
  • Support implementation of individualized service and treatment plans
    .
  • Connect youth and families to community resources and natural supports
    .
  • Coordinate care with internal teams and external providers (e.g., medical providers, schools, and state agencies).
Discharge & Aftercare Planning
  • Assist in planning for safe and stable discharge from residential care.
  • Coordinate and follow up on aftercare services to support continued success.
  • Serve as a liaison between families, treatment teams, and community partners
    .
Team Collaboration & Support
  • Actively participate in multidisciplinary team meetings and case conferences
    .
  • Communicate updates and concerns in a timely and professional manner
    .
  • Work in the residential milieu as needed to support program operations and youth care
    .
Crisis Response & Safety
  • Respond to crisis situations and coordinate appropriate interventions.
  • Monitor and address safety risks within the program environment.
  • Maintain training in CPI/de‑escalation, First Aid/CPR, and medication protocols
    .
Documentation & Compliance
  • Maintain accurate and timely documentation in Electronic Health Records and required systems
    .
  • Ensure compliance with HIPAA, agency policies, and best practices
    .
  • Track services, progress, and outcomes to support quality care.
What You Bring
  • Strong interest in working with youth in behavioral health or residential settings
  • Ability to coordinate services and manage multiple priorities
  • Strong communication, organization, and problem‑solving skills
  • Commitment to trauma‑informed, strengths‑based, and client‑centered care
  • Ability to work both independently and as part of a team
Qualifications
  • High School Diploma or equivalency certificate.
  • Valid driver’s license (Class C or
