About the Role
Adult Community Clinical Services (ACCS) is a Department of Mental Health funded service that delivers evidence‑based interventions to individuals diagnosed with mental illness or dual diagnosis. The Housing Coordinator supports the ACCS Housing First Principles, cultivating respectful partnerships with clients, families, and private landlords to maximize housing opportunities. The Coordinator creates treatment goals, addresses barriers such as CORI or legal issues, and supports individuals in moving into and maintaining permanent independent housing.
Key Responsibilities- Engage with and develop relationships with persons to establish treatment goals and identify housing barriers; assist in addressing legal or CORI issues that impede housing acquisition.
- Facilitate the transition of persons into permanent independent housing and support them in maintaining their residences.
- Assist individuals to secure rental subsidies through affordable housing programs.
- Collaborate with the ACCS Integrated Team to set treatment goals, determine housing and service needs, and eliminate housing barriers.
- Handle site visits, application processing, and lease negotiations; educate and support individuals to understand lease terms, subsidy requirements, and related benefits; prepare and inspect households during move‑in/out.
- High School Diploma, GED, or equivalent.
- Valid driver’s license, reliable insured vehicle, and acceptable driving record.
- Willingness to learn and adaptable to new opportunities.
