Veterans Inc. is a premier non‑profit organization dedicated to ending veteran homelessness and inspiring 100,000+ veterans and their families across New England to achieve long‑term self‑sufficiency. As a national leader in veteran services, we are seeking a dedicated Case Manager to provide comprehensive support to veterans residing in our emergency shelter and transitional housing programs in Worcester.

Location

Cambridge St. and Canterbury St., Worcester, MA.

Compensation

Salary: $26.44 per hour (full‑time). Direct deposit, annual bonus potential, and a comprehensive benefits package.

Responsibilities
  • Perform client intakes: conduct individual screenings, evaluations and comprehensive biopsychosocial assessments for assigned participants.
  • Identify housing barriers and evaluate participants’ unique skill sets and obstacles to secure long‑term, stable housing.
  • Deliver case management: provide direct intervention, supportive service outreach and individualized care coordination to encourage housing placement readiness.
  • Assist with essential navigation: guide participants through employment searches, benefit applications, medical or behavioral health appointments, and transportation coordination.
  • Build supportive relationships: establish trust‑based connections with veterans and their families to promote the highest level of independent functioning.
  • Coordinate ongoing services: meet regularly to provide feedback, monitor progress, and align daily activities with personal goals.
  • Maintain accurate documentation: prepare precise case records, progress notes and individualized service plans while executing ongoing data collection.
  • Ensure audit readiness: keep all participants’ files compliance‑ready and update key personnel on resident progress and program issues.
Qualifications
  • Bachelor’s degree in Human Services, Social Work or related field.
  • Minimum of three (3) years’ experience in program management, case management, substance abuse prevention, outreach, and/or services to the homeless or veteran populations.
  • Strong computer skills and knowledge of Microsoft Office Suite (Word, Excel, Outlook).
