Responsibilities- Develop, implement, and manage Individual Service Plans (ISP), Behavior Plans, Medication Treatment Plans, and Assessments within designated time frames.
- Provide training, modeling, and assistance in activities of daily living and health maintenance, including meal preparation, apartment maintenance, exercise, socialization, and use of public transportation/ADA.
- Offer case management to support individuals in developing independence and achieving goals set in ISP.
- Collaborate with Ascentria Leadership and Finance to ensure compliance with rep‑payee responsibilities such as Financial Training Plan, Funds Management Plan, annual reporting, and bill payment.
- Transport individuals to and from appointments and assist with obtaining public benefits (e.g., Social Security, Mass Health, ADA transportation, food stamps, fuel assistance).
- Assist individuals during transitions, including moving furniture or belongings and using personal vehicles for transport.
- Provide problem‑solving guidance and model appropriate social behavior.
- Act as an advocate for individuals at various community levels, evaluating advocacy effectiveness and adjusting as needed.
- Maintain regular communication with Program Manager, Clinical Coordinator, and DDS staff, and complete required paperwork by deadlines.
- Participate in staff, team, and PQI process meetings.
- Conduct all work in a manner that respects client rights, privacy, and dignity, and follows all contractual regulations.
- Represent Ascentria Care Alliance professionally and in appropriate business attire.
- Perform additional duties as assigned.
- Bachelor’s Degree with a minimum of 1 year of experience in human services; high school diploma with 10 years of direct care experience may be considered as an alternative.
- Licensed or eligible to be licensed by ASWB (LICSW, LCSW, LSW, or LSWA) within 6 months of hire (strongly preferred).
- Minimum of 2 years of experience in community‑based human services.
- Knowledge of the DDS ISP process and Charting the Life Course (strongly preferred).
- Ability to implement treatment plans, set limits, and manage client behaviors.
- Proficiency in writing accurate, professional documentation in legible English daily.
- Capability to set priorities and manage multiple tasks to deadlines.
- Experience working independently and as part of a multicultural team.
- Basic Microsoft Office computer skills.
- Flexible schedule to meet client needs.
- Ability to provide crisis intervention calmly and effectively.
- Excellent verbal and written communication skills.
