Salary Range: $25.00 To $30.00 Hourly
Job Summary:
The Case Manager is responsible for overseeing and coordinating services for members enrolled in a Group Adult Foster Care/Adult Foster Care program. This position ensures compliance with regulations while supporting members in maintaining safe, community-based living arrangements. The GAFC/AFC Case Manager works closely with caregivers, healthcare providers, and interdisciplinary teams to promote member health, safety, and independence.
Key Responsibilities:
- Conduct comprehensive initial and ongoing assessments in accordance with regulations
- Develop and maintain individualized GAFC/AFC care plans, including Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) support needs
- Complete required in-home visits (minimum frequency as required, typically monthly or bi-monthly based on level of care)
- Monitor caregiver performance and provide training, coaching, and support
- Ensure all program documentation meets compliance standards (e.g., progress notes, service plans, incident reports)
- Coordinate care with physicians, nurses, behavioral health providers, and other community-based services
- Collaborate with the GAFC/AFC Registered Nurse (RN) for clinical oversight and care plan updates
- Maintain compliance with regulations and audits
- Participate in interdisciplinary team meetings, case reviews, and quality improvement initiatives
- Respond to urgent situations and provide crisis intervention when necessary
Qualifications:
- Bachelor's degree in Social Work, Human Services, Psychology, or a related field required
- Minimum of 1-2 years of experience in case management, long-term care, or community-based services preferred
- Knowledge of GAFC/AFC regulations and regulation requirements strongly preferred
- Experience working with elderly individuals or adults with disabilities
- Valid Massachusetts driver's license and reliable transportation required
