Job Description Summary

The Solid Organ Transplant Coordinator is responsible for coordination of the evaluation and preparation of patients for organ transplant and long-term management of patients post-transplant. The Transplant Coordinator is responsible for coordinating patient care across the continuum through inpatient and outpatient settings. The Transplant Coordinator should demonstrate independent thinking through a high degree of problem solving and innovation. These skills are required to resolve complex problems that arise unexpectedly and apply appropriate interventions to individual patients while performing daily tasks.

The incumbent serves as a clinical resource for other team members, patients, and caregivers, referring providers, and community clinicians. Interacts daily with patients, families/caregivers, nurses, pharmacists, social workers, navigators, financial counselors, and physicians to provide and facilitate care. Provides direct patient care including education of patients and caregivers regarding transplantation including assessment, monitoring, and management of pre- and post-transplant side effects and complications.

CLINICAL DUTIES

  • Conduct pre-transplant interviews with prospective transplant patients and families. Synthesize and interpret eligibility criteria for patients, families, physicians and outside agencies.
  • Analyze pre-transplant clinical data and collaborate with other medical personnel to prepare patients and families for transplantation; gather and review medical and social history.
  • Coordinate pre-transplant preparations.
  • Coordinate evaluation process for pre-transplant (referral) patients to ensure effective use of hospital and clinical resources.
  • Educate patients and families on the transplant process.
  • Conduct post-discharge planning and education.
  • Provide direct care to transplant patients in the Outpatient Transplant Clinic.
  • Assess post-transplant organ function and general health status and collaboratively determine need for additional assessment or intervention.
  • Provide care in both the inpatient and outpatient setting to facilitate continuum of care through the entirety of the transplant journey.
  • Collaborate with other clinicians to facilitate patient care and ensure patient needs are met.
  • Utilize UNOS guidelines to select appropriate patients for organ transplantation in collaboration with transplant team.
  • Provide telephone consultation and follow-up to support patients and their families.
  • Implement and coordinate education, selection and preparation for live kidney donors.
  • Provide on-call service during off-shifts, weekends, and holidays on a rotational basis to coordinate activities prior to transplant and provide medical triage for outpatients in collaboration with the on-call physician.
  • Participate in the development and implementation of processes, policies, and standards of care for transplant patients in collaboration with the multidisciplinary team.

ADMINISTRATIVE, OUTREACH, AND EDUCATION DUTIES

  • Provide education for patients along the transplant continuum to assist in the understanding of processes and procedures.
  • Assist in onboarding/training new associates to the department.
  • Participate in relevant research and continuing education opportunities.
  • Monitor and input patient data in collaboration with the Data Analyst to meet UNOS requirements.
  • Complete necessary UNOS documentation to maintain compliance standards (listing, status changes, post-transplant follow up information).
  • Maintains relationship and collaboration with external community stakeholders to ensure holistic patient needs are met.
Minimum Qualifications

Bachelor's Degree:
Nursing (Required) RN - Registered Nurse - Ohio Board of Nursing
