Job Overview

Registered Nurse Home Health at Elara Caring. Provide high‑quality care to patients in their homes, advancing Elara’s mission of right care at right time in right place. Key Responsibilities
  • Ensure all activities align with Elara Caring’s vision and leadership expectations.
  • Maintain patient and staff privacy and confidentiality under HIPAA Privacy Final Rule.
  • Assess assigned case load to identify physical, psychosocial, and environmental needs.
  • Assume primary responsibility for caseload, including assessment, planning, coordinating, implementing, and evaluation of treatment plans.
  • Acquire data on physical, psychological, social, and spiritual factors and incorporate it into the plan of care.
  • Make initial evaluation and re‑evaluate nursing needs during each visit.
  • Conduct follow‑up visits based on patient, family, and facility needs.
  • Communicate significant findings, problems, and changes to the Clinical Supervisor, physician, facility, and other personnel.
  • Revise the plan of care in response to identified issues and notify relevant team members.
Required Qualifications
  • Current, unrestricted state RN license.
  • 1+ year of experience in a clinical care setting as a nurse.
  • Ability to lift, push, pull, and support up to 50 pounds.
  • Willingness to travel within the branch/office coverage area; dependable vehicle, valid driver’s license, and current auto insurance.
Preferred Qualifications
  • Associate’s or Bachelor’s Degree in Nursing.
  • Previous Home Health or Hospice experience.
  • Experience with Home Care Home Base.
