Well Be Senior Medical is seeking a highly skilled RN to deliver urgent, in-home care and telehealth consultations, collaborating with APPs and interdisciplinary teams to address patient needs across the care continuum.
You will perform assessments, coordinate care, educate patients and caregivers, maintain accurate records, and support care transitions and palliative care initiatives. Travel locally as needed with a 10am-7pm EST schedule.
