Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice Admission RN for a position in Wyandotte, Michigan.

• Collaborate with an interdisciplinary team to manage and deliver patient care plans.
• Provide direct patient care in hospice admissions.
• Maintain accurate charting and meet critical deadlines.
• Manage patient visits, including approximately ~20% driving time to or from patient locations.
• Self‑manage time and schedule.

• RN license in the state where you work.
• Associate degree or higher from an accredited School of Nursing.
• Two years of RN experience; hospice experience preferred.
• Current driver’s license.
• Demonstrated experience with tablets, mobile phones, and EMR software.
