Job Overview
We have an exciting opportunity for a full‑time Health Services Coordinator RN to work in Longview, Christian Health’s North Jersey premier assisted living residence. The Health Services Coordinator RN supervises and directs the functions of the Longview/Courtyard unit in accordance with federal, state and local Assisted Living standards, working closely with DOHS, administration and charge nurses to ensure the highest quality care for residents.
Responsibilities- Supervise and monitor LPN, CMA and PCA personnel, providing assignments and resident care instructions while overseeing breaks.
- Demonstrate understanding of Assisted Living principles: resident choice, privacy, independence and dignity.
- Show knowledge of aging, cognitive impairments, Alzheimer’s disease and other dementias.
- Ensure staff compliance with infection control and safety policies.
- Interact with staff regarding resident condition, give and receive nursing office reports at shift change, and follow up on issues to ensure resident well‑being.
- Oversee medication administration compliance per facility, pharmacy and nursing practice guidelines.
- Maintain resident records per policy, documenting treatments, medications, conditions, and family meetings.
- Implement and enforce resident rights, communicating complaints or grievances to DOHS/administrators and ensuring adherence to policies and laws.
- Communicate significant changes to families/residents, schedule service plan meetings, and document outcomes.
- Establish good working relationships with other departments and community agencies.
- Conduct daily rounds, talk with staff, and monitor resident treatment and medications.
- Perform nursing assessments per policy.
- Accurately transcribe physician orders and follow through (e.g., pharmacy calls, imaging, notifications).
- Report and recommend to DOHS/administrators regarding unit operations, and assist in coordinating nursing services with other disciplines.
- Schedule monthly wellness checks and ensure completion by LPN.
- Oversee monthly weight monitoring for each resident.
- Meet weekly with the team to discuss ideas, solve problems, and review care issues.
- Maintain records and oversee care to ensure compliance with Assisted Living regulations; participate in government agency surveys and inspections.
- Be available in person or by telephone for emergencies, providing on‑call coverage as needed.
- Ensure residents receive scheduled annual physicals and/or re‑certifications.
- Provide direct nursing care or assist with medication passes as necessary.
- Counsel, discipline and correct PCA/CMA personnel; complete performance evaluations and submit for approval.
- Coordinate with shift nurses to ensure staff understand each resident’s plan of care and that service plans are followed.
- Ensure initiation and completion of service plans and health care records.
- Participate in developing, maintaining and updating policies, procedures, manuals and objectives.
- Oversee residents’ admission to Longview in coordination with DOHS.
- Coordinate training for new staff with DOHS and scheduling coordinator.
- Record antibiotic use for each resident; inform Infection Control Nurse of communicable diseases.
- Return discontinued medications to pharmacy and ensure timely destruction of others per policy.
- Order medical supplies as needed.
- On admission, follow up on vaccine requirements (Mantoux, Pneumovac, Flu, Covid, RSV), obtain consent or declination, and keep records current.
- Communicate with pharmacy, reorder meds, manage refills, and maintain logs of pharmacy errors.
- Complete accident/incident reports per policy.
- Maintain list of current labs ordered by MD and schedule them; fax results to appropriate MDs.
- Provide leadership, direction, and support to nursing/CMA/PCA personnel assigned to unit/shift.
- Schedule MD appointments and medical tests, communicate results with MDs.
- Monitor resident conditions and alert physician/family to changes, obtaining orders as indicated.
- Work with DOHS to develop, implement and coordinate appropriate CQI/PI standards.
7 am – 3 pm, Monday – Friday. On‑call coverage for one weekend each month.
EducationMust be a graduate of an accredited school of nursing.
Qualifications- 3 years experience working with the elderly and 1 year supervisory experience preferred.
- Graduate of an accredited school of nursing.
- At least 21 years old.
- Obtain Train‑the‑trainer Certification within the first year with renewal every 5 years.
