We are looking for strong MDs located in the Western and Midwestern United States: WI, MN, IL, IA, CO, NV.

Position Summary

In this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our CCM, RPM, and BHI services.

Key Responsibilities
  • Establish the Tapestry Health patient relationship for our Chronic Care Management, Remote Patient Monitoring, and Behavioral Health services.
  • Be available at a minimum of 10 weekdays per month to conduct approximately 300-400 non-clinical judgment consults onsite and remotely. These are brief, non-clinical judgment consults, simply advising patients of the services they will get with Tapestry Health. No prescribing or treating involved.
  • Document patient encounters accurately and thoroughly within our platform at time of visit.
  • Provide end of day report to program administrators to track progress for completion of visits.
Minimum Requirements
  • Qualified MD/DO
  • Successful completion of US Residency in Internal or Family Medicine.
  • Active, unrestricted, Medical Licenses in at least 5 states.
  • Excellent clinical judgment and decision-making skills.
  • Must have reliable transportation to travel to assigned buildings.
  • Commitment to providing patient centered care and improving health outcomes.
  • Familiarity with electronic health record (EHR) systems and healthcare technology.
  • Ability to travel within 1-2 hours of assigned region (Midwest, United States).
Compensation Structure

10-25k/month, mileage reimbursement, Malpractice.
