We are looking for strong MDs located in the Western and Midwestern United States: WI, MN, IL, IA, CO, NV.
Position SummaryIn this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our CCM, RPM, and BHI services.
Key Responsibilities- Establish the Tapestry Health patient relationship for our Chronic Care Management, Remote Patient Monitoring, and Behavioral Health services.
- Be available at a minimum of 10 weekdays per month to conduct approximately 300-400 non-clinical judgment consults onsite and remotely. These are brief, non-clinical judgment consults, simply advising patients of the services they will get with Tapestry Health. No prescribing or treating involved.
- Document patient encounters accurately and thoroughly within our platform at time of visit.
- Provide end of day report to program administrators to track progress for completion of visits.
- Qualified MD/DO
- Successful completion of US Residency in Internal or Family Medicine.
- Active, unrestricted, Medical Licenses in at least 5 states.
- Excellent clinical judgment and decision-making skills.
- Must have reliable transportation to travel to assigned buildings.
- Commitment to providing patient centered care and improving health outcomes.
- Familiarity with electronic health record (EHR) systems and healthcare technology.
- Ability to travel within 1-2 hours of assigned region (Midwest, United States).
10-25k/month, mileage reimbursement, Malpractice.
