The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Cornerstone Healthcare, Inc., either directly or through coordination of care with other spiritual counselors.
The role includes spiritual assessments, care planning, direct spiritual care, liaison with community chaplains, documentation, and participation in interdisciplinary teams. It also recruits volunteers and conducts memorial services as requested.
