Schedule/Hours
  • Travel assignment, schedule details provided upon interview.
Qualifications
  • Active RN licensure in the state or compact eligible.
  • Minimum of one year of experience in Rehab or Long-Term Care nursing preferred.
  • BLS (Basic Life Support) certification required.
  • Strong assessment, communication, and documentation skills.
  • Demonstrated commitment to safe patient handling and safety protocols.
Responsibilities
  • Conduct comprehensive assessments on all patients within 8 hours of admission and formulate individualized plans of care.
  • Collaborate with patients, families, and the interdisciplinary team to identify needs and establish teaching goals.
  • Participate in team conferences and ensure timely, accurate charting of all care-related activities and observations.
  • Deliver direct patient care using the nursing process, with an emphasis on education and rehabilitation outcomes.
  • Initiate discharge planning at admission and provide ongoing patient and family education.
  • Communicate effectively with physicians, charge nurses, and ancillary staff to ensure continuity and quality of care.
  • Supervise and provide leadership to ancillary nursing personnel; delegate responsibilities appropriately.
  • Maintain environmental and personal safety, while promoting a positive and efficient work atmosphere.
  • Assume charge responsibilities as assigned and proactively pursue professional development.
