Licensed Practical Nurse (LPN) – Continuing Care (Night Shift)
Location:
Grand Rapids, MI 49503 (On-site)
Schedule:
12-hour nights, 7:00 PM–7:30 AM , every other weekend (no on-call) Pay: $28–$35 per hour Scrubs:
Navy Blue Summary Now hiring a Licensed Practical Nurse (LPN) for a Continuing Care / Long-Term Care setting in Grand Rapids, MI. If you’re a dependable LPN who thrives on night shift and takes pride in accurate documentation, patient safety, and teamwork, this is a strong opportunity with consistent hours. In this Licensed Practical Nurse (LPN) role, you’ll provide direct patient care under the direction of an RN, mid-level provider, or physician and collaborate with the care team to support safe, respectful, high-quality care.
Responsibilities Support patient assessment, care planning, implementation, and evaluation in coordination with RN/provider Provide direct patient care consistent with unit routines, policies, and procedures Communicate with patients/families about treatments, medications, and equipment as appropriate (in coordination with RN/provider) Perform accurate clinical documentation/charting in the patient record (computerized and/or written) Collaborate with the interdisciplinary team to ensure continuity of care and coordinated services Maintain a clean patient environment/work area;
report equipment issues/malfunctions as needed Document chargeable items per standards Participate in quality monitoring activities and support safety/risk-reduction initiatives Follow privacy, confidentiality, and data security expectations; complete required competencies on time Perform other duties as assigned Qualifications Active, current, unrestricted Michigan LPN license (required) High School Diploma/GED and completion of an approved nursing program (required) Successful completion of a competency-based orientation program (required) BLS certification (American Heart Association or American Red Cross) within 90 days (required) Experience in long-term care (LTC), skilled nursing facility (SNF), continuing care, rehab, or nursing home settings (preferred) Comfortable with EMR/EHR charting , fast-paced workflows, and team communication Reliable attendance and ability to work nights + every other weekend *
