Under the direction of the Practice Leadership, provides patient care within specialty and procedural service lines requiring advanced clinical competencies and technical skills. Performs patient assessment, administers treatments and medications, assists with procedures, and supports diagnostic testing. Conducts telephone triage using established protocols and documents care in the medical record. Coordinates care across the continuum, serving as a liaison with providers and external partners.
Applies the nursing process to promote optimal patient outcomes and collaborates with the care team to ensure effective care coordination. May provide guidance and direction to clinical support staff.
- Bachelor of Science in Nursing (BSN), preferred.
- Current State of Michigan License (unrestricted) as a Registered Nurse (RN)
- Current Basic Life Support (BLS/CPR) required.
- HPE Infusion/Cancer Center Infusion RN:
Must obtain ONS/ONCC Chemotherapy Immunotherapy Certificate prior to administering chemotherapy. - Cardiovascular Physician Practice: ACLS, CV-BC required within 1 year of hire.
- Wound: ACLS, HBO certifications required within 1 year of hire.
- Previous outpatient experience and/or experience in specialty preferred.
- Excellent interpersonal, verbal and written communication skills
- Exhibits creative problem solving and critical thinking skills
- Possesses excellent time management, coordination and organization skills
- Delivers clinical therapeutic services in the office setting appropriate for all ages
- Ability to triage and manage complex clinical issues utilizing assessment skills and protocols
- Ability to work independently as well as in a team environment
- Capable of adapting to changes in health care with the goal of improving quality, efficiency and cost effectiveness of care

- Conducts telephone triage according to established clinical protocols educating patients and guiding them to appropriate care.
- Maintains an active nursing appointment schedule for assisting providers in the management of patients with acute and chronic conditions.
- Identifies patient care requirements by completing the nursing process.
- Provides condition specific patient education and anticipatory guidance under the direction of providers.
- Assists care manager and provider in follow-up management of post-hospitalized or post emergency room visits.
- Screens patients for care opportunities and collaborates with treating provider to identify referrals to care management.
- Communicates and coordinates with care team on clinical issues related to patient care.
- Administers medication and treatment according clinical protocols and provider oversight.
- Document concise, accurate and timely information in the patient care record, and completes forms associated with patient care.
- Establishes a compassionate environment by providing patients with emotional support and comfort.
- Protects patients and employees by adhering to infection prevention and control standards.
- Maintains and protects patient confidentiality.
- Perform efficiently in emergency patient situations following established standards, remaining calm, informing appropriate persons, and documenting events.
- Maintain a current knowledge base and use standards of care in professional practice.
- Assume responsibility for own practice including appearance, attendance, punctuality and flexibility in scheduling.
- Pursue self-directed learning experiences, continuing education, and seek direction when necessary.
- Participate in peer review, quality assurance, standards development, and research activities.
- Demonstrate awareness of legal issues in all aspects of patient care and department function and strive to manage situations in a reduced risk manner.
