Role:
Hospice Registered Nurse (RN)
Responsibilities:Provide home-based care to patients with a prognosis of six months or less, focusing on improving quality of life. Collaborate with an interdisciplinary team including physicians, LPNs, CNAs, social workers, chaplains, and volunteers to execute care plans. Conduct patient assessments, monitor vital signs, evaluate breathing, manage pain, and administer medications and treatments. Ensure medical equipment and supplies are accessible. Educate families about prognosis, care plan changes, medications, and hospice goals.
Take a holistic approach addressing physical, emotional, spiritual, and family needs.
Valid, active RN license in Michigan. Minimum of one year of relevant RN experience, with community health and Medical-Surgical background preferred. Reliable transportation, valid driver’s license, and current insurance required.
