Hospice RN – Weekend On-Call

Role: Registered Nurse in hospice care, responsible for providing patient-centered care and support within a flexible, weekend-only schedule.

Key Responsibilities:

• Conduct ongoing assessments of patients’ physical and psycho-social status
• Develop and adjust care plans collaboratively with the Interdisciplinary Group, patients, families, and physicians
• Educate patients and families, manage pain and symptoms, and evaluate responses to treatments
• Ensure availability of necessary equipment, supplies, and medications
• Maintain accurate documentation and participate in IDG meetings

Qualifications & Skills:

• Minimum of one year of relevant RN experience
• CPR certification
• Reliable transportation, valid driver’s license, and insurance
• Preferred: management and home health experience, Medical-Surgical background
