Sublette County Health operates two clinics with urgent care services, level 5 trauma receiving emergency rooms, two EMS stations, and a newly opened county hospital. Heritage Home, our skilled nursing facility, is expanding to a 40‑bed skilled nursing unit and a 10‑bed memory care unit. We seek dedicated professionals to join our team and build the future of healthcare in Sublette County.

Description

The Skilled Nursing Facility Certified Nursing Assistant (CNA) provides outstanding patient care and physical support for residents’ Activities of Daily Living (ADL). The CNA utilizes knowledge of residents’ rights, legal/ethical concepts, communication skills, safety, and infection prevention while performing basic nursing skills, restorative skills, mental health and psychosocial skills, and nursing team member skills.

Supervision Received

Reports to the Director of Nursing (SNF).

Responsibilities
  • Participates in patient and resident care within scope of training, in accordance with state and federal regulations and facility policies.
  • Assists in preparation for resident admissions and prepares rooms for new admissions.
  • Accompanies residents during admissions, transfers, and discharge procedures.
  • Provides age‑appropriate morning care, including bed bath, shower or whirlpool, oral hygiene, hair combing, back care, dressing, changing bed linens, cleaning over‑bed tables, and maintaining room cleanliness.
  • Provides evening care, including hand/face washing, oral hygiene, back rubs, peri‑care, linen freshening, and room straightening, following the resident’s Plan of Care.
  • Notifies licensed staff when a resident complains of pain.
  • Provides post‑mortem care.
  • Assists RN/LPN/LVN with treatments and procedures.
  • Uses proper body mechanics and resources to assist with positioning, lifting, turning, ambulation, and operating specialized equipment, monitoring for skin breakdown and reporting to the staff nurse.
  • Performs all resident care aspects in a safety‑oriented environment to reduce errors.
  • Supports a culture of safety and quality.
  • Records vital signs, height and weight, intake and output, and other data as assigned, reporting variances to staff nurses.
  • Prepares residents for meals, distributes trays, aids or feeds residents as needed, respecting dietary restrictions, and offers snacks and hydration.
  • Ensures residents are dressed and clothes are stored properly, keeping them clean and dry.
  • Records observations of behavior, complaints, physical symptoms, and reports issues to the staff nurse/CNA Manager, noting changes in status, care response, unusual occurrences, assignment issues, or safety concerns.
  • Participates in development, implementation, and evaluation of resident care plans, protecting residents from neglect and abuse.
  • Understands restraint policies and proper use.
  • Accompanies residents to appointments as directed.
  • Provides general activities to residents in isolation.
  • Actively receives shift reports, knowing resident locations.
  • Rounds with the incoming shift to ensure a clean unit and accurate resident status.
  • Promotes resident rights, increases quality of life, and involves residents in care decisions.
  • Encourages self‑care and independence with ADL interventions as noted on each care plan.
  • Supports rehabilitation goals and implements documentation as required.
  • Attends staff development activities to maintain competency and certification.
  • Treats residents and families with respect and dignity, addressing psychosocial, cultural, ethnic, and spiritual needs, acting as liaison among administration, physicians, and other providers.
  • Adheres to departmental policies, performance improvement, safety, environmental and infection prevention standards.
  • Demonstrates flexibility, organization, and composure under stress.
  • Maintains personal health to prevent illness transmission.
