Warm Valley Health Center is seeking a skilled and compassionate Clinical Nurse/Case Manager to join our fast-paced outpatient team in Fort Washakie, WY. • Perform patient triage, assessments, and administer urgent care treatments

• Deliver medications, IV therapy, and assist in minor procedures

• Conduct EKGs, lab draws, and prepare patients for diagnostic exams

• Coordinate care plans and referrals with an interdisciplinary team

• Provide chronic care support, case management, and patient follow-up

• Lead wellness initiatives and community health education outreach

• Maintain accurate and timely EHR documentation, ensuring compliance

• BSN required

• Active Wyoming RN license

• BLS certification (ACLS/PALS preferred)

• Experience in outpatient, urgent care, or case management

• Certification in Case Management is a plus (or willingness to obtain within 1 year)

• Excellent critical thinking and triage judgment

• Skilled in care coordination and patient advocacy

• Culturally responsive with a heart for community health

• Confident working both independently and in integrated care teams

• Familiarity with social determinants of health and outreach resources

• Applicants must submit a copy of Tribal Enrollment card or CIB for Indian Preference.

Preference will be given to a qualified Eastern Shoshone tribal member, then other qualified federally recognized Indian tribal members and then other qualified candidates.

Veterans who meet the minimum qualifications and provide documentation of an honorable discharge (DD214) from any branch of military service are entitled to receive preference points during the interview process.
